All of our other services will be discounted at 15%.
The plan costs $320 annually per person, with a $50 discount for every family member added to the plan.

This is a dental discount plan and not dental insurance. Offer good only at Seasons Dental in Burley, Idaho. Offer is non-transferable and may not be combined with any other dental insurance or promotions.
Should there be a dental treatment needed following any type of injury where a lawsuit and therefore outside medical, cat, disability or workman’s comp type insurance are involved, this discount plan cannot be used.
Payment is due at time of service. Dues are non-refundable. Refunds will not be given if patient chooses not to use their dental plan. Family members cannot be substituted for another family member. Cleanings must be performed before the end coverage date. Children must be under the age of 18, unmarried and live in the same household. Family members can be added at anytime during the year, however, the end coverage date will remain the same for all members.
Patients agree that Seasons Dental fees must be paid at the time services are rendered. Plan prices, terms and conditions are subject to change.
